The Southwestern District Health Unit is part of the Million Hearts Grant pilot project. The Million Hearts Grant aim statement is by June 30, 2015, through coordination and collaboration, identify and treat hypertensive patients between ages 18-85 in North Dakota by linking public health and clinical systems which include EMS and Community paramedics, using data, to improve care coordination and standardization.
The Southwestern District Health Unit part in the project is to provide blood pressure screenings at oil industries or oil related industries in Southwestern North Dakota, to identify diagnosed or uncontrolled high blood pressure.
